Procedure Overview
Buttocks augmentation at Truffel Healthcare can be achieved through two methods: autologous fat grafting (the Brazilian Butt Lift or BBL) or silicone buttock implants. The BBL uses the patient's own fat harvested through liposuction from the abdomen, flanks, lower back, or thighs, then strategically injected into the buttocks to add volume and enhance curves. Silicone implants are an option for patients who lack sufficient donor fat, though they require prefabrication (pre-ordering based on measurements) before the procedure.
The BBL and liposuction are performed together and billed separately. VASER technology is employed during fat harvest, achieving up to 90% fat retention due to preservation of fat nano-particles. Because the procedure uses your own biological tissue, results look, feel, and move naturally, without risks associated with implants such as capsular contracture or displacement. Dr. Matwa follows internationally recognised safer BBL protocols, which have significantly reduced procedural risk compared to older techniques.
Downtime is 4 to 6 weeks. A BBL pillow and mattress are used to avoid pressure on the grafted fat during the critical early healing period. Silicone implants do not reduce in size over time but require removal or exchange after approximately 10 years.

How It Works

Under general anaesthesia, tumescent fluid is infiltrated into the donor areas. Liposuction is performed using fine, atraumatic cannulas specifically designed to minimise trauma to the harvested fat cells - a critical factor for fat survival rates post-grafting.
The harvested fat is processed to remove tumescent fluid, blood, and non-viable cells, isolating a purified, concentrated graft. This step is time-sensitive and handled with meticulous care to preserve fat cell viability before reinjection.
Purified fat is injected at multiple levels within the subcutaneous buttock tissue using blunt-tipped cannulas to maximise fat survival and safety. Fat is never injected into the muscle to eliminate the risk of fat embolism. Volume is distributed to achieve the planned shape and symmetry.

What to Expect
Patients must avoid sitting directly on the buttocks for the first 2-3 weeks. A special BBL pillow is provided - pressure on the newly grafted fat impairs blood supply and reduces survival rates.
Compression garment worn over donor areas. Gentle walking allowed. Swelling and bruising are normal. Follow-up visit for wound check. Rest on the stomach or sides only.
Graduated return to sitting using the BBL pillow. Light desk work possible. Donor areas soften noticeably. Transferred fat cells begin establishing their blood supply during this critical window.
Fat retention settles - typically 60-80% of injected volume survives. The final shape, projection, and symmetry are now fully visible. Exercise resumes gradually. Results are stable and long-lasting.
