Patients must avoid sitting directly on the buttocks for the first 2-3 weeks. A special BBL pillow is provided - pressure on the newly grafted fat impairs blood supply and reduces survival rates.

BBL results are long-lasting, provided weight is kept stable. Once the transferred fat cells survive - which is confirmed by 3-6 months post-operatively - they behave like the body's other fat cells. Significant weight loss after surgery will reduce the volume of the transferred fat, while weight gain will increase it proportionally, just as it would elsewhere in the body. Sustained pressure on the buttocks during the first 2-3 weeks compresses the delicate new capillary networks forming around the grafted cells, depriving them of oxygen and nutrients and causing cell death. Candidates need sufficient fat in the donor areas - abdomen, flanks, lower back, or thighs - to harvest an adequate volume for meaningful augmentation. Dr. Matwa assesses this at your consultation. Very lean patients with minimal body fat may not be suitable candidates for a BBL, as there may not be enough donor fat available. Attempting to harvest fat from a very lean patient risks contour irregularities at the donor site without providing enough volume for the buttocks. A body fat percentage of at least 22-25% is generally preferred.
